When you come for your appointment please remember to obtain and bring the following:

• Drivers License or a valid ID
• Insurance information
• Referral Letter (if required)
• Reports, X-rays, MRI's, CT scans etc and any other relevant information
• List of medications (if any)

If you need to cancel an appointment

Telephone the office during business hours and allow at least 1 day's notice so that we can offer your appointment time to patients on our waiting list.

We recognize that your time is valuable, and we make every effort to run on time. Occasionally emergencies or patients require a little more time, and these cause scheduling delays beyond our control. We apologize if we keep you waiting.

New Patient Appointment Scheduling
(You will need to do the following)

• To make an appointment please call our main number (512) 450-1300
• Office Hours are Monday to Friday: 8:00am to 5:00pm
• Please have your insurance card available so that we can help you obtain the optimum benefit. We are contracted with most managed care plans
• We promise to always try to provide you with a convenient appointment time. If you require same day or next day care we will do our best to accommodate you while honoring our professional responsibilities to all patients
• To save time, you may print and complete the required new patient forms on-line prior to your appointment. If not, Please arrive 20 minutes prior to your scheduled appointment time to complete the required forms in our office
• If your insurance requires a referral from your primary care physician, please contact your doctor to request the referral. Many offices require 24-48 hours to process the referral and forward it to us
• Please have your insurance card and driver’s license available
• If you have x-rays, MRIs or other imaging studies related to your injury, please bring them to your appointment. This will allow us to avoid duplicating the service and related cost
• We will collect co-payments, as required by your insurance plan, at the time of your appointment. We accept cash, checks and all major credit cards

Our offices are on the northeast corner of W. 38th and North Lamar at 900 W.38th, Suite 300. We are located in the Central Park Medical Building just south of the Heart Hospital of Austin and Central Market.

Parking is available in the Central Park Garage located on the behind (east side) of the Hospital. Garage parking fee is $1.50 per hour.

Handicapped access, automatic doors and valet assistance is provided at the front entrance.
